Comprehensive midwifery care for Mrs. Y, aged 31 years, G2P1A0H1, was provided continuously through the Continuity of Care (CoC) approach to monitor the condition of the mother and baby from pregnancy through the postpartum period. At the initial assessment, Mrs. Y was at 41 weeks and 3 days of gestation with a physiological pregnancy and a complaint of abdominal pain radiating to the lower back. The care was planned and provided through pregnancy monitoring, education on danger signs and birth preparedness, first to fourth stage of labor care, newborn care, neonatal visits (KN1-KN3), and postpartum visits (KF1-KF4), with SOAP documentation. The method used was a case study with data collected through interviews, observation, physical examination, and documentation review at PMB Darliana, S.Keb Pekanbaru from November 2025 to October 2026. The results showed that the pregnancy and labor progressed physiologically. The baby was born spontaneously, cried strongly, was active, received Early Initiation of Breastfeeding (IMD) and essential newborn care, and remained in good condition with age-appropriate growth during KN1-KN3. The postpartum period progressed physiologically; however, at KF2, breast milk production was not optimal and the nipples were less prominent. After lactation massage was provided, breast milk flow became smoother and the mother felt more comfortable. Continuous comprehensive care supported the monitoring of maternal and infant conditions and the early detection of problems. Mrs. Y is expected to continue health monitoring, pay attention to reproductive health, and use the chosen contraceptive method according to her needs.
