Abstrak :

Asuhan kebidanan komprehensif atau Continuity of Care (CoC) merupakan asuhan kebidanan yang diberikan secara berkesinambungan mulai dari masa kehamilan, persalinan, bayi baru lahir, masa nifas hingga keluarga berencana. Asuhan ini bertujuan memantau kondisi ibu dan bayi, mendeteksi faktor risiko serta komplikasi secara dini, dan memberikan pelayanan sesuai kebutuhan. Tujuan penelitian ini memberikan asuhan kebidanan komprehensif pada Ny. R di PMB Bdn. Silvi Ayu, S.Keb Pekanbaru mulai dari masa kehamilan, persalinan, bayi baru lahir dan masa nifas. Metode penelitian ini adalah studi kasus dengan pendekatan Continuity of Care. Subjek penelitian adalah Ny. R usia 28 tahun G3P1A1H1. Pengumpulan data dilakukan melalui wawancara, pemeriksaan fisik, dan dokumentasi. Asuhan meliputi dua kali kunjungan kehamilan, asuhan persalinan kala I sampai kala IV, tiga kali kunjungan neonatus, dan empat kali kunjungan nifas. Pendokumentasian dilakukan dalam bentuk SOAP. Pada masa kehamilan ditemukan keluhan nyeri pinggang yang ditangani melalui edukasi dan pijat ibu hamil. Persalinan berlangsung spontan dan fisiologis tanpa komplikasi. Bayi lahir dengan berat badan 3.900 gram dan meningkat menjadi 4.100 gram pada usia sembilan hari. Pada masa nifas, ASI ibu belum lancar, dan tubuh ibu pegal sehingga diberikan asuhan melalui edukasi, pijat laktasi, dan pijat nifas. Kondisi ibu semakin membaik, ASI semakin lancar, dan rahim mulai kembali ke ukuran semula sesuai dengan proses pemulihan masa nifas. Pada akhir masa nifas, ibu memilih kontrasepsi suntik tiga bulan. Secara keseluruhan, asuhan kebidanan komprehensif terlaksana secara berkesinambungan sesuai kebutuhan ibu dan bayi.

Comprehensive midwifery care, or Continuity of Care (CoC), is midwifery care provided continuously from pregnancy, childbirth, and newborn care through the postpartum period and family planning. This care aims to monitor the conditions of the mother and baby, detect risk factors and complications early, and provide services according to their needs. This study aimed to provide comprehensive midwifery care to Mrs. R at the Independent Midwifery Practice (PMB) of Midwife Silvi Ayu, S.Keb., in Pekanbaru, covering pregnancy, childbirth, newborn care, and the postpartum period. The research method was a case study using the Continuity of Care approach. The subject was Mrs. R, aged 28 years, G3P1A1H1. Data were collected through interviews, physical examinations, and documentation. The care included two antenatal visits, intrapartum care from the first to the fourth stage of labor, three neonatal visits, and four postpartum visits. Documentation was recorded using the SOAP format. During pregnancy, the mother reported lower back pain, which was managed through education and pregnancy massage. Labor occurred spontaneously and physiologically without complications. The baby was born weighing 3,900 grams, increasing to 4,100 grams at nine days of age. During the postpartum period, the mother experienced perineal suture pain, abdominal cramping, delayed breast milk flow, and body aches. Care was provided through education, lactation massage, and postpartum massage. The mother's condition gradually improved, breast milk production became more abundant, and uterine involution progressed physiologically. At the end of the postpartum period, the mother chose the three-month injectable contraceptive. Overall, comprehensive midwifery care was provided continuously according to the needs of the mother and baby.
